Items of Extraordinary Value (Form 1023)

Declare goods worth more than $100 per pound, certify the list at origin, and acknowledge receipt at destination.

Form 1023 lists the customer's high-value goods: items worth more than $100 per pound. The driver asks the customer, types each item, and the customer certifies the list is true at origin and acknowledges receipt at destination.

Important: This form protects the customer. If a high-value item isn't listed and the form isn't signed, the carrier's liability for that item is limited to $100 per pound. The exact warning is quoted below and prints on the form.

Who this is for: The driver, walking the high-value conversation with the customer on move day.

Open the form

  1. Open the interstate job and scroll to Move-day documents.
  2. Tap Items of Extraordinary Value. The screen opens with the title Extraordinary Value.

The Autofilled from this job section shows Order #, Customer, and Agent #. Its footer reads: "Items of extraordinary value are goods worth more than $100 per pound. These print on the form automatically."

When there are none

If the customer declares no high-value items, turn on No items of extraordinary value. Its footer: "Turn on if the customer declares no items worth more than $100 per pound." When it's on, the item list is hidden and the PDF prints the NONE line:

"NONE — no items of extraordinary value are included in this shipment."

List each high-value item

With the toggle off, the Items of extraordinary value section appears. The section footer reads: "The driver asks the customer and types each item. Photos are optional." Before any rows, it shows "No high-value items listed yet."

  1. Tap Add item.
  2. Fill the fields:
    • Article — placeholder "Article — e.g. Diamond ring, 2ct".
    • Condition at origin — placeholder "Condition at origin — e.g. No visible wear".
    • Security seal # and Declared value — side by side.
  3. Optionally tap Add photo. Once attached, the row shows a thumbnail and "Photo attached"; tap the trash icon to remove it.

Note: A row with nothing typed and no photo is dropped when the form saves. Swipe left to delete a row yourself.

Signing at origin

The At origin section holds an Agent / Driver # field and signature rows for the Customer and the Carrier representative. On screen its footer reads "The customer certifies the list above is true, correct and complete."

The customer's origin certification prints verbatim on the PDF:

"I certify that I have read and understood the forgoing form and affirmatively state the above listed information to be true, correct and complete."

Signing at destination

The At destination section has its own Agent / Driver # field and signature rows. Its on-screen footer reads "The customer acknowledges receipt of the items at delivery."

The destination acknowledgment prints verbatim on the PDF:

"All items containing the contents listed above have been opened, and I certify and acknowledge receipt of the above items and further state that the items inside arrived safely and in the same condition as at origin."

The definition and liability warning on the PDF

The PDF opens with the definition, verbatim:

"Items of extraordinary value are defined as those items having a value greater than $100 per pound."

Followed by the limited-liability warning, verbatim:

"If for any reason, items having value in excess of $100 per pound per article are included in your shipment and you fail to list those items on this inventory or fail to sign this inventory, you expressly understand and agree that the carrier's liability for loss or damage to those items will be limited to no more than $100 per pound per article (based upon the actual article weight)."

Status and export

The form's badge turns Signed once the customer signs at origin (the move-day artifact is complete). The destination signatures come at delivery and don't push it back to Draft.

Tap Export PDF to render the form on your company's letterhead. Tap Done to save and close.

Common mistakes

  • Skipping the form on a high-value shipment. If items aren't listed and signed, the customer's protection drops to $100 per pound. Fill it out or use the "No items" toggle deliberately.
  • Leaving declared value blank. Type the value the customer states so it prints exactly as entered.
