Pre-Existing Damage (Form 1002)

The packer's log of pre-existing damage on carrier-packed contents, acknowledged by the customer before the property is packed.

Form 1002 is the packer's record of pre-existing damage on carrier-packed contents — the chipped wine glass, the cracked plate found while filling a carton. It is not for furniture and not for the home. The customer signs to acknowledge those conditions existed before the property was packed.

Important: This form covers contents only — the small goods that go into boxes. Furniture pre-existing damage is recorded on the item itself in the inventory. Damage to the residence goes on Residential Property Damage (Form 1025).

Who this is for: The packer, working the boxes on move day.

Open the form

  1. Open the interstate job and scroll to Move-day documents.
  2. Tap Pre-Existing Damage. The screen opens with the title Pre-Existing Damage.

Autofilled from the job

The top section, Autofilled from this job, shows read-only fields the app already holds: Order #, Customer, Origin, and Agent #. The footer reads "These print on the form automatically." You don't retype them.

Who packed it

Under Packed by, type the packer's name in the Packer name field. It prints in the Packed By cell at the bottom of the form.

Add damaged contents

Each row is one damaged article and the condition observed.

  1. Tap Add item.
  2. Fill the two fields:
    • Article — for example, "Wine glass, crystal" (the field placeholder reads "Article — e.g. Wine glass, crystal").
    • Condition — for example, "Chip on brim" (placeholder "Condition — e.g. Chip on brim").
  3. Repeat for each damaged item.

The section header reads "Pre-existing damage on packed contents." Its footer reminds you: "Note contents with damage found while packing — the chipped glass, the cracked plate. Furniture and the home are recorded elsewhere."

Before anything is entered, the section shows "No damaged contents noted yet."

Note: Any row you leave completely blank (no article, no condition) is dropped when the form saves, so empty rows never reach the PDF. To remove a row yourself, swipe it left to delete.

The customer signature

The customer's signature acknowledges the listed conditions before packing.

  1. In the Signature section, tap Customer signs.
  2. The Customer Signature sheet opens. Its footer reads: "The customer's signature acknowledges the pre-existing conditions listed above exist before the property is packed."
  3. Enter the Customer name, sign, and tap Done. See Signing move-day forms.

Once signed, the row shows "Signed by" the customer's name with the date and time, and the button changes to Re-sign.

The legal language on the PDF

The exported PDF reproduces the paper form's instructional and acknowledgment text verbatim. The two introductory paragraphs read:

"This form is to be filled out by the packer(s) on all interstate shipments. It shall indicate those items to be packed which have pre-existing damage or are incomplete. This will enable both the customer and the carrier to be aware of articles that have partial damage or repairs which are obvious to both parties before they are packed."

"The customer, or a designated representative, shall indicate with his or her initials that they have observed the condition of the item as described by the packer. This form does not take the place of the inventory, but shall be read in conjunction therewith. All items (whether they appear on this form or not) are subject to a post-move inspection by a representative of the carrier in cases where a claim for loss or damage is filed on an interstate shipment."

The acknowledgment above the signature line reads:

"Each article listed above has been called to the attention of the customer and by signature of the carrier's representative and the customer, and they acknowledge these conditions exist at the time the property is tendered for shipment."

Note: The printed form has a Customer Initials column. It is left blank in the app and initialed by pen, per the form.

Export the PDF

Tap Export PDF in the top-right. The form renders on your company's letterhead and the share sheet opens. Tap Done in the top-left to save your work and close the form.

Common mistakes

  • Logging furniture here. Form 1002 is for contents that get boxed. Record furniture damage on the item in the inventory instead.
  • Logging home damage here. Walls, floors, and banisters go on Form 1025.
  • Forgetting the packer name. It prints in the Packed By cell — fill it in.
